Is the instrument available?

I found this article interesting and a potentially valuable contribution to the field of Health Literacy. However, I was disappointed that the instrument was not included in the article. It is very difficult to understand the strengths and weaknesses of a survey instrument that you can't look at. Is the instrument available?
